provide to us. Any personal information that Sudbury Laser Health Clinic retains is kept in such a

receiving LLLT treatment consult your physician/dermatologist/or Health Care Professional

You are hereby granted permission to access the Content from the Site in whole or in part, solely for
your personal use of viewing and browsing through the Site. This permission terminates

Terms and conditions of Service
Set out below are the terms on which The Sudbury Laser health Clinic provides service to its clients.
Please ensure that you have read and fully understood them prior to booking any treatment.
1. Your time is very valuable to you and to us. We do not double-book the appointments and we do
our best to run on schedule, so you don’t have to wait when you come in to our office. If you miss an
appointment or do not cancel or reschedule it with at least 24-hour notice, we will charge you $75
per hour of blocked time for your treatment. For after-hours, Saturdays or Holiday appointments, the
fee is $150 per appointment. In the event that the equipment is down for weather, power outages or
for any reason, we reserve the right to reschedule the appointments. We will work with you to
reschedule the appointments at a convenient time for you, in case of an emergency on your part.
2. Credit card details, deposits, refunds and payments
Credit/debit card details are taken in respect of consultation and pre-assessment bookings in order
to secure the appointment. No fee will be charged to this card unless you fail to attend your
appointment or cancel it with less than 24 hours' notice. Under these circumstances, we will make
a charge of $55 or $150 per appointment for after-hours, Saturdays or Holiday appointments, this
includes follow up appointments
All treatment bookings must be paid for in full, prior to any treatment commencing. LLLT is
considered a treatment option, and therefore there are no guarantees. No refunds will be offered on
any treatments. If you have paid for a treatments or a course of treatments in advance and are
subsequently unable to complete it, we may offer you a refund. No refunds will be granted once first
session has commenced. Any refunds will be refunded to the means in which they were paid. Any
refunds on treatments paid in advance are at the discretion of the Registered Manager. Refunds
may take up to 90 Days
3. Courses of treatments
All treatments purchased as a course must be paid for in full in advance of the first treatment. All
treatments must be taken within 12 months of the date of purchase; any treatments left untaken
after 12 months will be forfeited.
4. Treatment suitability
We will always assess whether treatment is suitable for you, or likely to be successful, prior to any
treatment being carried out. If not, we will inform you as to the reasons why.
5. Liability
The Sudbury Laser Health Clinic will not be liable in contract, tort or otherwise for any economic loss
(including, without limitation, loss of profit), or for any other special, indirect or consequential loss or
damage arising out of, or in connection with, its provision of any goods and/or services to the client.
It is the client's responsibility to ensure that he or she provide The Sudbury Laser Health Clinic with
all relevant medical details prior to each treatment. The Sudbury Laser Health Clinic will not be
liable for any damage that occurs as a result of the client's failure to disclose such details.
The client agrees to comply with all instructions and/or recommendations given to them by, or on
behalf of, The Sudbury Laser Health Clinic regarding the care of any treated area. Nothing in these
terms of business shall exclude or limit The Sudbury Laser Health Clinic liability for death or any
personal injury resulting from The Sudbury Laser Health Clinic negligence.
6. Children
The Sudbury Laser Health Clinic is not registered to treat clients under the age of 18. All children
under the age of 18 who visit the premises must be accompanied by an adult on a 2:1 ratio at all
times and be respectful of the restful nature of the clinic environment. Any children accompanying an
adult will not be allowed access to the laser room before, during or after treatment. Children will be
required to wait in the reception area, however must have an accompanying chaperon. Unfortunately
The Sudbury Laser Health Clinic staff cannot be responsible for observing children during
treatments. It is therefore advisable that clients make alternative arrangements for child care during
their treatments.
8. Miscellaneous
All prices are correct at time of printing and are subject to change from time to time. The Sudbury
Laser Health Clinic reserves the right to change, amend, alter or withdraw treatments without prior
notification.
9. Complaints Procedure
The Sudbury Laser Health Clinic endeavors to treat all its clients appropriately, compassionately
and fairly. If, however, you have an issue with any matter in relation to your treatment at The Sudbury
Laser Health Clinic, you are entitled to lodge a complaint, either verbally, by telephone or in writing.
11. Special Offers
From time to time The Sudbury Laser Health Clinic may produce promotional offers. These offers
may be withdrawn at any time by the registered manager. Terms & Conditions for special offers are
listed in the clinic and can be confirmed upon request with the registered manager.
